Posted On: 10 OCT 2022 6:09PM by PIB Chennai

ESIC Medical College and Hospital, K. K Nagar, Chennai is a 550 bedded hospital providing Primary, Secondary and certain Super Specialty Medical Care for the Insured Persons and their dependants under ESI Scheme.  As a part of expanding the healthcare services, ESIC Medical College & Hospital, KK Nagar, Chennai has started Super speciality services in the departments of Cardiothoracic Surgery, Paediatric Surgery, Neurosurgery, Rheumatology in addition to the already available facilities of Medical Oncology, Oncosurgery, Urosurgery and Vascular Surgery. This will immensely benefit the IPs and ESIC beneficiaries in the state of Tamil Nadu.

Recently, the hospital has done two rare surgeries one involving a pulmonary disease in a 23 old male and another surgery for a preterm baby born at 29 weeks of gestation with congenital abnormality. A brief details of the surgeries are as detailed below,

Pulmonary Surgery:

Restrictive lung disease in young adults causes breathlessness during normal physical activities. We operated upon a 23-year-old male diagnosed with Pleural disease suffering from poorly expanding Right Lung secondary to tuberculous hydropneumothorax. The Cardiothoracic Surgeon Dr. Senthilkumar did right thoracotomy for decortication of affected pleural segments. He was assisted by Dr. Lohithsai, General Surgeon.

General anaesthesia is required for such major surgery along with additional challenge of   One Lung Ventilation, which is intermittent collapse of diseased lung and exclusive ventilation of normal lung, carrying risks of desaturation and hypoxia. If not intervened at right time, it may result in disastrous complications. Anaesthesia team headed by Prof. Dr Ilango Ganesan handled the patient with specially designed Double lumen tubes and Fiberoptic bronchoscopy to enable one lung ventilation. He was assisted by Prof Dr Radhika, Dr Deepak and Dr Sundaram. Meticulous care was provided by the anaesthesia team during surgery with good postop recovery.

Post operatively the patient was kept pain free in post anaesthesia care unit with Erector spinae plane nerve block catheter. On post-operative day one, patient was able to take feeds and feeling comfortable to breath with expanding lungs on both the sides.

Surgery for congenital abnormality on preterm baby:

A preterm baby born at 7 months (29 weeks gestation period) born with congenital abnormality (imperforate anus) was taken up for surgery (diversion sigmoid colostomy) in ESIC Medical College, KK Nagar, Chennai. The baby was born with caesarean section operation and was weighing just 1.3 kg. Early correction of this congenital abnormality was required. The baby was operated in the second day of birth by our newly joined paediatric surgeon Dr Apurva and plastic surgeon Dr Vishwanathan. Operating such preterm and small baby is a challenge for both the surgery and anaesthesia teams. As such small for date babies do not have well developed organs, it is a challenge for the anaesthesia team to administer anaesthesia drugs. Also in view to very low birth weight all the anaesthesia medications have to be given meticulously with careful monitoring. Also surgery itself is a stress over and above the preterm condition of the baby. Small changes in anaesthesia medications or fluids can have drastic effects on the baby.

The anaesthesia was administered by Dr Ilango Ganesan, Dr Radhika, Dr Deepak R,          Dr Sundaram, Dr Ashwini, Dr Arun Prakash, Dr Praveen and postgraduate residents of Anaesthesia Department. ICU care and postoperative care of the baby was provided by the paediatric team Dr Shobhana, Dr Aparna, Dr Prameela and paediatric team.

The above surgeries demonstrate the achievements of the institute in the domains of neonatal surgery and anaesthesiology.
