Rare 15 Kg Retroperitoneal Liposarcoma Removed Successfully at JIPMER

A major complex oncological surgery was successfully performed at the Department of Surgery, Unit-I, JIPMER on a 73-year-old male patient, a resident of Kallakurichi, Tamil Nadu. The patient was admitted on 2026 May 16 with a diagnosis of a giant Retroperitoneal Liposarcoma.
He presented with complaints of abdominal distention for 1 year and shortness of breath for 6 months. Evaluation revealed a large predominantly fat containing mass, compressing the left ureter and engulfing the left kidney. In view of the ureteric involvement, the Department of Urology performed left ureteric DJ stenting on 19 May. Subsequently, the patient underwent open surgical excision of the tumor on 20 May. The surgery was performed by Dr. Raj Kumar Nagarajan, Professor, Department of Surgery, along with his team comprising Dr. Vimalraj (Senior Resident), Dr. Shubham (Junior Resident), and Dr. Siddu (Junior Resident) and Anesthesia team was led by Dr Sivashanmugam (Consultant).
The procedure commenced at 10:30 AM and concluded at 3:00 PM. Intraoperatively, the tumor was found to be infiltrating the left kidney, necessitating left nephrectomy along with complete excision of the tumor in toto. The excised tumor measured approximately 65 × 45 cm and weighed 15 kilograms, which is third largest in India and second largest in south India (30.5 Kgs by Mahamana pandit Madan Mohan Malaviya Cancer Centre, Varanasi in 2023 and 30.2 Kgs by Yashodha Hospital, Hyderabad in 2022).
Postoperatively, the patient is being managed in CCU- ward 21. He was successfully extubated on postoperative day 1 and is presently hemodynamically stable.
The successful management of this rare giant retroperitoneal tumor highlights the advanced surgical expertise available at JIPMER in managing highly complex surgical conditions.
