Shri D.K.Singh, Commissioner of Railway Safety, (South Central Circle, Secunderabad), Ministry of Civil Aviation, Govt. of India conducted a statutory inquiry into the Derailment of Locomotive and 3 coaches of 14266 DN Dehradun-Varanasi Janata Express hauled by Locomotive No. 11129 WDM3D/AMV at sand hump on loop line No. 1 of Baccharawan station at Km. 1025/3-5 on Lucknow-Raibareli section of Lucknow Division of Northern Railway at 09.11 hours on 20.3.2015. As a result of the incident, 39 passengers were killed and 38 passengers were injured out of which 24 were grievously injured.

            According to the provisional findings of the Commissioner appended with his preliminary report, the incident occurred due to “Error in Train working”. These findings are under the consideration of the Government.
